Service Options Using Resources in Community Environment (SOURCE)

CSS Healthcare is an approved provider of state funded Medicaid services. The private home care program allows the participants to receive care in the comfort and privacy of their own home instead of hospital or nursing home placement. Individuals that are SOURCE eligible are referred by Case Managers to CSS Healthcare. After a referral, a member of our qualified nursing staff will complete an assessment which helps to determine how much care an individual needs. An individual’s care plan will be based upon the person’s need for assistance with functional tasks or medical monitoring. Individuals must be Medicaid eligible and meet the nursing home level of care needs in order to receive SOURCE approved services.

Community Care Services Program (CCSP)

CSS Healthcare Services is state approved to provide community based services to elderly or functionally impaired persons. In order to receive CCSP, individuals must meet the medical, functional and financial eligibility criteria for placement in a nursing facility. All CCSP recipients must be Medicaid eligible and be approved by a physician.

Services provided under the SOURCE/CCSP program may include:

• Personal care and hygiene
• Dressing, grooming, bathing
• Food Preparation and feeding
• Ambulatory assistance (cane, walker, wheelchair)
• Short errands
• Oral Hygiene
• Light housekeeping
• Assistance with Medication
